The Safe Senders list contains the email address or domain.
I noticed it happening with our *.world domain, which isn't *.com or a usual / familiar domain, and which ends in 5 characters after the last dot... could this be the reason something is failing behind the scenes ?
This has been a problem for years and it remains a problem. WHY, Microsoft?
Doesn't work. Email addresses added to safe sender list are not saved.

Manually adding junk senders to safe sender list is annoying, why hasn't MS fixed this bug yet?
Eliminate "Safe Senders" list, it is constantly, incorrectly, interfering with routine business communications. It serves no useful purpose.
